TSB released its investigation report (R23Q0022) into a train operated by Quebec North Shore and Labrador (QNS&L) that passed a signal displaying a Stop indication on its way to Mai Station, Quebec, in February 2023. The investigation found that despite the signal being visible 0.75 miles away, the locomotive engineer did not undertake the braking manoeuvres required to stop before the signal. About 50 feet from the signal, the locomotive engineer realized that it was displaying a Stop indication and applied full dynamic braking and the emergency brakes. There was no collision or derailment, and there were no injuries. The investigation found that there have been 12 occurrences, including this one, of QNS&L trains passing Stop signals in the last 10 years. Following the occurrence, the TSB issued a safety information letter to QNS&L on the supervision of newly qualified locomotive engineers. As a result, QNS&L made changes to its evaluation program and increased the frequency of evaluations by supervisors in the field. Vessels navigating the Red Sea have seen war risk premiums surge following intensified attacks by Houthi rebels, though Chinese and Russian-linked vessels have received discounts due to public Houthi assurances that their vessels won't be targeted. But on Saturday, the Chinese-operated Suezmax tanker, Huang Pu, was hit by five Houthi anti-ship ballistic missiles. While the damage sustained was minimal and no casualties were reported, the attacks occurred days after reports the Houthis had reached a more formal agreement with China and Russia for the safe passage of their ships. This development underscores the presence of risk despite the agreement and is likely to prompt further evaluation of risk assessments for Chinese and Russian-linked vessels. https://lnkd.in/g4rUAwhx

Canada commits to buying 12 new conventionally-powered, under-the-ice #submarines - a massive acquisition with numerous #shipbuilders from the around world already eyeing the program reported to be worth at least $60 billion Canadian dollars. Canada wants a new class of submarines to replace its four aging Victoria-class boats. The list of interested #shipbuilding companies include Saab’s new C71 ‘Expeditionary’ sub (Sweden), and Thyssenkrupp Marine Systems' 212CD class sub (Germany).
