Fascinating new research from Dr. Jeralyn Bernier Jacobs, a physician research scholar at Mid-Atlantic Permanente Medical Group | Kaiser Permanente, shows a significant rise in the incidence of #prediabetes and #TypeIIDiabetes in children and adolescents early in the COVID-19 pandemic. CHLA Los Angeles County Children's Health Survey 2024 This survey aims to get your opinions about community health issues and the quality of life in Los Angeles County for children and families. Children's Hospital Los Angeles (CHLA) will use the survey results in its community health needs assessment (CHNA) to help identify the health needs and the factors that influence the health of Los Angeles (LA) County children and families. CHLA will use information from the CHNA to inform their community health improvement plan so that resource investments can address the most pressing needs identified by the community.   CHLA has engaged Health Management Associates (HMA) to conduct this survey to assure community residents that their feedback is anonymous. Show me some progress for 40% of the population in 2621 counties lowest in health care workforce forever with just 25% of the primary care workforce paid less to the tune of less than 20% of primary care spending. This works out to less than 50 billion. Show me at least 10 billion more on the way to the 100 billion plus needed so that these practices can have sufficient team members and health professionals to integrate, coordinate, and do patient centered are. I want proof of funding changes - no promises, no articles, no reports Don't care if this goes to CHCS, small practices, medium size practices, whatever - but anything less is a violation of CMS guidelines, health equity, and Belmont principles that we must apply to health care design the same as with physicians and human subject researchers.

CONCLUSIONS Small independent practices serving vulnerable patients in urban communities are surviving because deeply committed physicians are making personal sacrifices. Health equity–focused policies could decrease the burden on these physicians and bolster independent practices so that socially vulnerable patients continue to have options when seeking primary care. Yes, the financial design does not work for primary care where the public plans and weakest employers are concentrated - or if your population is elderly and poor. Yes, our nation does not have the social and other supports needed for Americans older and more behind. Yes, our financial design is closing and compromising practices and hospitals specific to most Americans most behind - a 40 year unbroken run of policy changes. We can all name colleagues that once made a difference but were forced to make changes because of the design. Many of us in urgent care, emergent, or hospitalist workforce are there because of the debts we ran up trying to do the right thing - what we trained in primary care to do. America is about 100 billion a year short in primary care spending and spending 100 billion more would be a best practices method for addressing health equity along with similar investments in mental health, womens health, geriatrics, and basic surgical for the 40% of the population with half enough of each basic health access workforce. There is no excuse for CMS 1.4 trillion a year designed against primary care and basic health access and 2621 counties lowest in health care workforce. The 40% behind in 2621 counties lowest in health care workforce are joined by at least 10% behind in counties with concentrations of workforce - that are failed by their health plans in multiple ways that defeat access. The current design is the opposite of health equity making the designers, associations, foundations, and institutions into deceivers when they claim to be health equity focused and yet have not accomplished the necessary distributions of basic health access funding. Why does Medicare pay less as levels of workforce get lower? Why do we let CMS get away with claiming that its plans cover the costs of delivering care when they do not and especially when CMS adds more costs and more duties and more toxicity to environments home and practice? https://lnkd.in/g2FxeJiH

This month, Kaiser Permanente hosted a Health Action Summit which proved to be a testament to the power of collaboration in transforming healthcare. With leaders including DC Mayor Muriel Bowser, Dr. Mandy Cohen, and U.S. Department of Health and Human Services (HHS) Secretary Xavier Becerra driving the conversation, we delved into the critical need for a stronger emphasis on preventive and primary care within our health systems. Despite the fact that only a small fraction of US healthcare spending is allocated to prevention, the consensus was clear: there must be a shift from an exclusive focus on treating illness to also focus on preventing disease. This approach not only promises better health outcomes for individuals but also represents a more sustainable model for our nation's healthcare system. The passion and alignment was palpable. Not only are midwives being challenged by health care systems to practice to the full scope of their education and licensure, but even growing a workforce of midwives is now significantly diminished by UCSF School of Nursing mandate to move the historic masters graduate midwifery program to a DNP. Why? This article hit the San Francisco Chronicle today! I hope the readership and citizens of CA can understand significant barriers exist to not only practice midwifery, but now to become a midwife, particularly those from underrepresented groups. Once again we see the ramification of profit over purpose. There are NO published studies that demonstrate value for cost as an entry degree to practice in midwifery, or any other advanced practice provider role. I urge heightened pressure for this decision to be reversed and reinstate a historic and predominantly BIPOC faculty remain as educators. The dismantling of the midwfiery program is beyond reason and provides suspect to many inherent struggles within the SON. #Enough #GrowMidwives

Today is Latinx physician day. California is approaching 40% Hispanic population and yet has only about 6% of Latino/Latina physicians. Less than 2% physicians identify as Mexican American .The Latinx population faces more healthcare disparities and poorer health outcomes. Studies show that if the physician speaks Spanish , there are less emergency room visits and better outcomes with asthma care. We need to do more to help support and promote more Latinx physicians in California and the US.
