El Centro de Corazón provides services to the uninsured, underinsured, and to those unable to afford health and dental care. Individuals and families may qualify for a sliding fee scale discount based on income and family size. Medicare, Medicaid, Children’s Medicaid, CHIP, and CHIP Perinatal are accepted at each of its five health centers.
Bring the following documents to apply for Sliding Fee
Scale Discount Program:
(1) IDENTIFICATION
(bring one of the following for each related adult/each child in the household)
Adult: driver’s license, ID card, passport/visa,
immigration documents
Child: birth certificate, school record
(2) PROOF OF INCOME
(bring at least one of the following)
Last month’s paycheck stubs, welfare benefit documents (TANF, food stamps), Social Security documents, tax returns, Medicaid Form
PROOF OF ADDRESS
Mail with your name, lease or house payment, voter
registration, utility bill
