The Hebrew Hospital Home Continuum of Care is a not for profit, nonsectarian geriatric center offering a full continuum of care ranging from 120 modern, private condominiums for independent seniors at Westchester Meadows, to the most intense level of nursing care, at Hebrew Hospital Home of Westchester. Whether you need care in your home or in our home we are a leading elder care services provider. The Continuum of Care serves more than 2,000 older people in greater New York.

Since its founding in 1928, the Hebrew Hospital Home has grown to include a long-term residential health care facility, a leading Medical Model Adult Day Health Care Program, a Home Care program serving over 1000 patients in the community and a Managed Long Term Care Plan, HHH Choices Health Plan.

Voted as US News and World Report for Most Outstanding Nursing Homes, Hebrew Hospital Home of Westchester and Fieldstone at Westchester Meadows have been honored again in 2011.
