Oxford Health Plans, Inc. through its subsidiaries provides health benefit plans primarily in New York, New Jersey, and Connecticut in the United States. The company offers commercial products, including large group, small group, individual, and health maintenance organization (HMO), as well as Medicare products to different types of customers. Its product line includes HMO plans, exclusive provider organization (EPO) plans, point-of-service (POS) plans, preferred provider (PPO) plans, indemnity plans, and various plans offered to Medicare beneficiaries. The HMO plans provide comprehensive health care benefits through the company’s participating network providers. Its POS plans combine the benefits of HMO plans with certain of the benefits of the company’s indemnity health insurance by covering services provided by non-participating providers. Oxford Health’s PPO plans allow members to obtain coverage for services from participating providers or from non-participating providers. The company also offers self-funded health plans to employers who wish to retain the risk for health care costs; indemnity-type plans; and ancillary and specialty products, including dental, vision, long-term care, and work and family benefits, flexible spending accounts, and health reimbursement accounts. As of December 31, 2003, Oxford owned and operated three HMOs and two insurance companies. In addition, the company also provides individual and group annuity, and life and health insurance policies. Oxford Health markets its products through direct sales representatives, business representatives, the Internet, telemarketing representatives, and executive account representatives, as well as through external insurance agents, brokers, and consultants.
