A closed-loop insulin delivery system (look it up if you don't know what it means) -- and I do believe it will happen.

Of course, a cure for Type 1 diabetes would make that product obsolete, but I'm a bit skeptical that such a cure would be available in my lifetime.

Hand-held medical scanner, using a small wand with thousands of narrow beam RF transceivers (imagine each transmitter and receiver having it's own yagi antenna pointed downward but toward each other at about 30 degrees). Density of transceivers should be about 600 per inch. Accelerometer keyed to data stream to align speed and rotation. Plugs into laptop via USB where program analysis data in near realtime and renders images like an X-ray only colored based on reflectivity or other criteria selected. Great for doing a fast scan to detect bone breaks or fractures. For kinda an idea of what I'm talking about, see Aliens movie (third I think) where she's in a prison and scans herself to find the alien inside.

Inventions that I'd like to see:

1) Bio-replacement technology. Essentially a way to create or grow new organs or even a whole new body as needed. I envision this via a process akin to cloning, such that tissue rejection was not an issue. It would be much better if this was cheap and easy, but I'd settle for just making is practical.

2) Clean, cheap, and practically unlimited energy. There are several theories and some experimentation on how this could be done.

3) FTL (faster than light) travel. This is a pipe dream at the moment, but if we can solve the energy problem, I think this will fall pretty fast. Once we achieve FTL, we will no longer be limited to a single solar system.

Of course to the obvious, the cure for the diseases I would like to see someone invent a truth meter (something that you could put in the pocket of your coat like a pen and it would later tell you how honest that person in a particular meeting actually was).

A cure for cancer or other diseases? Could, controversially, cause overcrowding on this planet.
A time machine? Massive disruption to the space-time continuum. ;-)

There are so many possible and controversial answers here, and in an ideal world it would be nice to have the ability to chosse exactly what we want.

But, if we could find a way of making an truly 100% efficient engine, then that would go a long way to solving the constant petrol/diesel/gas price increases.

As that it pretty unlikely, then I would opt for a teleporter, making commuting a whole lot easier!

Clarification added February 3, 2008:

Sorry... should have been 'choose' not 'chosse'
