Where can I get small quantity, customized prepaid Visa/MC cards?

In searching for this so far, I have found customized solutions that involve only large quantities (10k). Are there are providers of smaller (10-100) customized cards?

I recognize that this may require a company to act as a broker or wholesaler for the larger card providers, but I would expect to pay a premium for small quantities. For those who feel that there may be value in that opportunity, here is exactly what I would like:

1. Customized card design. I'd like to select an image for the card background.I would like to create a customized design for each individual card, but would settle for one design. The card holders name should be on the card, unless this is prohibitively expensive.

2. Small Quantities. I don't expect to order more than 10-50 cards in a year.

3. Cost. I would expect to pay $10 per card in a single design, or upwards of $25 for each individually designed card. I expect to load them with $50-$100.

4. Refillable. If I'm going pay $10-25 for a card, I'd like it to be reusable. If there is a charge for refilling, I would want that billed to me rather than the cardholder.
