I have written a brief questionnaire on the use of professional networking sites like LinkedIn and It would be invaluable If you could click a few boxes! It requires no personal details. Finished survey. One recommendation, psychologically speaking, a survey of base judgement values should have at least 7 options to properly measure a value ranking.

SurveyMonkey is a pretty good tool for web-based surveys. It's inexpensive and displays surveys pretty well to respondents.
