I am sure you will get many responses from my people on the list as PM's. I will try to provide several to you in an unbiased fashion.
Most of the large MR suppliers will have a telephone (as well as internet) omnibus capability.
Some of those include Synovate, TNS, Ipsos, and ICR (Media, PA). I would recommend any of these, but I know for a time (and don't know if it is still true) synovate was using their panel for the omnibus vs. RDD. The main reason to do this was to save dialing time as RDD completion/response rates have dropped dramatically the past 5-8 years and there has been needs for CATI companies to try to find ways to save monies and reduce costs to clients.
If you don't want to work directly with a large MR house, you can try direct data collection companies such as Western WATS, Universal Surveys, etc. Both of those are very good organizations and you can let them know I referred you.
If you require any contact names to the above, I have worked for/at many of the organizations and can likely refer you to somone directly.
And one last note, many smaller CATI facilities can likely run you a custom "omnibus" at rates that are very competitive to a regular omnibus and balance it on the standard factors (age, gender, income, HH size, ethnicity, etc.) and you would be able to add several more questions since you would be paying on survey length and not on number of questions.
