Could anyone provide me with a sample employee satisfaction questionnaire/survey?
I need to design and implement an employee satisfaction survey for my company and this is my first experience at it. I am looking for a sample questionnaire or survey that i could use to compile my own. Any inputs/ advice / suggestions/ references will be great!
Also It would be very helpful to understand how these surveys should be executed and how should the answers received be analysed.

Look for GE's employee survey templates on the web. They are supposed to be really good.

What does your company value? This is the key question to start with.
What you measure will determine the answers received. If your company does not value what you're asking, the answers you get could be from employees that are a bad company fit. These answers are not something that should be acted upon, and can lead to confusion in employees about what the company culture is. ie asking training questions, when the company policy has no formal training
Start with the corporate employee strategy/culture and design questions that judge how well this is being implemented.

You may want to view some of the survey templates offered by Zoomerang. Zoomerang is an inexpensive online survey tool with extensive templates and help features.
I believe they have an employee satisfaction template and it is very easy to customize.
They also offer a free option, so you can create and distribute the survey to 100 respondents at no cost (it's about $20 a month if you need to distribute to more people).

I strongly urge you to invest in the employee engagement survey through Gallup. It's based on the book "First Break All The Rules" and it only covers 12 questions. You may want to read the book first and then learn how there is no more important thing to successful employee relations than to engaging them.

Hi Parul, you probably read this in many of the responses you have received, but Gallup is world leader in the domain of employee engagement. Read some of the books like 'First Break all the rules' or '12: Elements of Managing People' . Gallup' research in this area has been pathbreaking and you will find loads of research online. Do revert back if you need any specific info.
