I like to use a congruence model which takes a close examination of varying aspects of the organization. There's clearly a lot more to it but here's the short answer!
Break it out into different pieces such as:
strategy, organizational structure, processes, people, culture, management, customer, products and environment. Identify the difference between the current state and the desired state. Determine desired state by using benchmark data on best practices as well as the companies strategic vision. Use this information to develop the vision, mission and values. Develop plan to reach desired state in all areas and you're on your way to organizational excellence!
